Service Description / Overview

Wimmera Community Options offers a range of flexible community based services: Linkages, Community Aged Care Packages and Carer Respite Centre. Our services are provided throughout the Wimmera region to frail older people, persons aged 0 – 64 years of age with a disability. Carers are also eligible for assistance.

An assessment of each persons needs is required to determine eligibility. Wimmera Community Options provides case management, coordination and provision of additional services, if required, when these are not available through other services or programs. These services are complimentary, and enhance the range of support options offered by other service providers.

Wimmera Community Options believes that people, irrespective or age or disability, have the right to choose to remain at home as long as possible, supported by a range of integrated and flexible services that promote their independence and dignity, and contribute to their physical, emotional and social well being.
