Service Description / Overview

Services Include:

To assess older people and those with disabilities in order to help them and their carers make informed decisions regarding their individual care needs.

In consultation with the aged person's doctor, arrange hospital admission or other treatment/rehabilitation as necessary including nursing home or hostel placement.

In consultation with other health, welfare, voluntary and local council agencies, arrange for the provision of services and assistance to enable older people to continue to live in their own homes.

Offer advice on preventing falls or illness plus living safely and independently in your own home.

An assessment usually occurs at the person's home or other residence/hospital. A friend or relative is welcome to attend the assessment.
